Club T-Shirt order form

New members of GoBush are entitled to ONE FREE shirt, which includes having your “handle” (nickname) embroided on it. Additional shirts for other family members can also be ordered at a cost of $25 per additional shirt. To order your free shirt, and any additional shirts you need, you must submit the form below by the due date. If ordering additional shirts, then you must also deposit the appropriate amount of money into the club bank account (details below) on the same date as you fill out this order form.
